چکیده
The association between infections occurring in the first 2 years of life and development of eczema was investigated in 1,782 control children from a national population-based case-control study in the United Kingdom conducted over the period 1991–1996. Dates of eczema and infectious diagnoses were ascertained from contemporaneously collected primary care records. Children diagnosed with eczema before the age of 2 years had more prior clinically diagnosed infections recorded than did children without eczema (rate ratio 1⁄4 1.26, 95% confidence interval (CI): 1.18, 1.36). The difference in infection rates between children with and without eczema was apparent from birth and throughout the first 2 years of life. As expected, compared with children of second or higher birth order, those firstborn were at increased risk of eczema (p 1⁄4 0.020); however, the relation between eczema and prior infection was evident only among children of second or higher birth order and not among firstborn children (rate ratio1⁄4 1.45, 95% CI: 1.32, 1.59, and rate ratio1⁄4 1.08, 95% CI: 0.98, 1.20, respectively). The authors’ results are consistent with the notion that the association between birth order and eczema is unlikely to be attributable to variations in early infectious exposure.

Infections in asthma and allergy.
The role of infection in asthma is presently a paradox. While respiratory syncytial virus, among others, has long been believed to instigate immunological changes which lead to asthma, and rhinovirus might be an important contributory factor in the exacerbation of asthma symptoms in children and adults, there is increasing speculation that infections in early life have a protective role in the ...
متن کاملThe association of early life exposure to antibiotics and the development of asthma, eczema and atopy in a birth cohort: confounding or causality?
BACKGROUND In general, studies reporting positive associations between antibiotic exposure and respiratory and allergic disease have been unable to determine the nature of this association. OBJECTIVE To examine the association between antibiotic exposure in infancy and the development of asthma, eczema and atopy in early childhood. METHODS In a birth cohort study, we collected reported anti...
متن کاملEffect of sibling number in the household and birth order on prevalence of Helicobacter pylori: a cross-sectional study.
BACKGROUND Infection with Helicobacter pylori (H. pylori) is acquired mainly in childhood, with studies demonstrating this is related to living conditions. Effects of sibling number and birth order on prevalence of infection have not been extensively studied. METHODS The authors performed a cross-sectional survey of adults, aged between 50 and 59 years, previously involved in a community-scre...
متن کاملSiblings, multiple births, and the incidence of allergic disease: a birth cohort study using the West Midlands general practice research database.
BACKGROUND The presence of older siblings reduces the risk of developing hay fever, eczema and atopy, but findings for asthma have been inconsistent. Whether twins have a reduced risk of allergic disease is also unclear. We have investigated these questions in a birth cohort analysis of the West Midlands General Practice Research Database (GPRD). METHODS Our birth cohort included 29,238 child...
متن کاملEffects of antibiotic exposure and immune system challenge on the development of allergic asthma
In previous studies, experimenters have found that the risk of atopic disease is increased due to less exposure to infection and increased antibiotic use during immune system development. The data have supported this finding in accordance with hay fever, eczema and atopy, but are inconclusive regarding asthma. This study aims to further research the effects of antibiotic use and immune system c...
